I live and work abroad
Your right to Dutch health insurance will expire. It is necessary that you cancel your health insurance with us. Use this cancellation form to do so. You can insure yourself in the country where you live.
I'm going on secondment abroad
We are not able to determine whether you should remain insured in the Netherlands. The Social Insurance Bank (SVB) determines whether you are entitled to Dutch health insurance. Contact the SVB to clarify your insurance situation and that of your family members.
How to proceed?
If the SVB states that you are obliged to maintain health insurance in the Netherlands, you can request an A1 statement from the SVB. Send an email with the statement attached. Your insurance will then continue.
If the SVB indicates that you are not allowed to remain insured in the Netherlands, you can request a 'Wlz statement' from the SVB. You can then cancel your insurance with us. Send an e-mail with the Wlz statement as an attachment. You can then register with an insurer in the country where you live and work.
I work abroad and live in the Netherlands
Your right to Dutch health insurance will expire. It is necessary that you cancel your health insurance with us. Use this cancellation form to do so. You can register with an insurer in the country where you work.
To ensure that you can receive care in the Netherlands, it is important to register with the Dutch health insurer CZ Verdragen. CZ Verdragen asks you for an S1 form from the country where you work.
